Anterior cruciate ligament (ACL) injury is a prevalent musculoskeletal condition that significantly affects individuals beyond physical impairment. In addition to pain, instability, and functional limitations, ACL injuries are associated with psychological distress, fear of re-injury, reduced confidence, and altered social participation. These multidimensional consequences have a substantial impact on overall wellbeing and recovery outcomes. Behavioral factors such as motivation, self-efficacy, coping strategies, and adherence to rehabilitation play a critical role in influencing physical recovery and long-term health following ACL injury. This chapter explores the impact of ACL injury on physical, psychological, and social wellbeing from a behavioral health perspective. It examines key behavioral determinants affecting rehabilitation engagement and discusses the application of behavioral and bio-psychosocial models in ACL management. The role of physiotherapy-led, patient-centred interventions in promoting positive health behaviors and holistic wellbeing is highlighted. Understanding the behavioral aspects of ACL rehabilitation can enhance treatment outcomes, support sustained return to activity, and improve quality of life. The chapter emphasizes the need for integrating behavioral research into clinical practice and future rehabilitation strategies.Published
