Abstract

Ashwagandha (Withania somnifera), an important herb in Ayurvedic medicine, has gathered increasing scientific attention for its broad-spectrum therapeutic effects. Traditionally recognized as a Rasayana, Ashwagandha is known for promoting vitality, longevity, and mental clarity. This chapter presents a comprehensive review of the phytochemical composition, pharmacological activities, safety profile, and clinical relevance of Ashwagandha. Bioactive compounds such as withanolides, withaferin A, and withanosides contribute to its adaptogenic, anti-inflammatory, antioxidant, and neuroprotective properties. Human clinical trials and preclinical toxicological studies consistently demonstrate its safety across a wide dosage range, with minimal and transient adverse effects. Mechanistically, Ashwagandha modulates key biological pathways, including the HPA axis, SIRT3 signalling, and NLRP3 inflammasome, making it a promising candidate in the management of stress-related disorders, neurodegeneration, hypothyroidism, reproductive health, cardiovascular, and respiratory diseases. The herb has also shown immunomodulatory and antiviral potential, particularly during the COVID-19 pandemic. Furthermore, emerging evidence highlights its role in dermatological applications and metabolic regulation. Multiple pharmacokinetic studies emphasize its bioavailability and tolerability in humans. Despite its wide use in complementary and integrative medicine, Ashwagandha remains underrepresented in mainstream clinical guidelines. This chapter explores the need for standardized formulations, long-term safety evaluations, and translational research to establish the role of Ashwagandha in modern healthcare.
