Abstract

Water quality in dialysis units represents a critical cornerstone of patient safety, given that hemodialysis patients are exposed to approximately 300–400 liters of water per session. Inadequate water treatment can introduce microbial contaminants, endotoxins, chemical impurities, and heavy metals directly into the bloodstream through the dialysis membrane, causing acute and chronic adverse outcomes including hemolysis, septicemia, aluminum toxicity, and cardiovascular complications. This chapter provides a comprehensive examination of water treatment systems employed in dialysis facilities, encompassing the stages of pre-treatment, reverse osmosis, deionization, and distribution loop management. It discusses the physicochemical and microbiological standards mandated by international regulatory bodies, including the Association for the Advancement of Medical Instrumentation (AAMI) and the International Organization for Standardization (ISO). Quality control protocols, routine monitoring procedures, and corrective action frameworks are analyzed in detail, with emphasis on microbial surveillance, chemical analysis, and endotoxin testing. The role of staff training and interdisciplinary collaboration in maintaining water quality standards is also explored. Emerging technologies such as ultraviolet irradiation, real-time online monitoring, and advanced membrane filtration systems are reviewed. By synthesizing current evidence and best practices, this chapter underscores that rigorous water treatment and quality assurance programs are indispensable for optimizing patient outcomes, reducing treatment-related morbidity, and ensuring regulatory compliance in modern dialysis units.
