Abstract

Renal replacement therapy (RRT) has transformed the management of acute and chronic kidney failure, significantly improving patient survival and quality of life. Over the years, hemodialysis, peritoneal dialysis, and kidney transplantation have evolved with remarkable technological and clinical advancements. However, existing modalities still present considerable challenges, including high treatment costs, limited accessibility, cardiovascular complications, infections, poor patient adherence, and reduced quality of life. The future of renal replacement therapy is therefore focused on patient-centered innovations, precision medicine, wearable and implantable artificial kidneys, regenerative medicine, stem cell therapy, xenotransplantation, artificial intelligence integration, and sustainable dialysis practices. Emerging developments in nanotechnology, biomaterials, tissue engineering, and bioartificial organs are opening new possibilities for restoring kidney function more physiologically and effectively. Additionally, home-based dialysis systems and telemedicine are reshaping the delivery of nephrology care by increasing convenience and reducing healthcare burden. This chapter explores the future perspectives in renal replacement therapy, emphasizing current limitations, recent scientific advances, technological innovations, and the potential of next-generation therapies to revolutionize kidney disease management. The discussion also highlights ethical, economic, and global healthcare considerations associated with future RRT modalities.
