Abstract

Pain is a complex Sensory and emotional experience that alerts body to possible or actual damage, acting as a vital defense mechanism. Pain is now seen as a complex combination of peripheral, spinal, supraspinal, psychological, and environmental elements rather than only a biological phenomenon. For physiotherapist, a thorough understanding of pain anatomy and physiology is essential for clinical reasoning, evidence-based evaluation and rehabilitation planning. The anatomical arrangement of peripheral nociceptors, spinal cord routes, ascending and descending pain systems, nociception processes, pain modulation, peripheral and central sensitization, and neuroplasticity related to chronic pain are all covered in this chapter. By focusing on therapeutic exercise, manual therapy, patient education, and movement-based rehabilitation, it further emphasizes the clinical value of pain neurobiology in physiotherapy. Physiotherapy can provide customized treatments that enhance function, lessen impairment, and improve patient outcomes for musculoskeletal, neurological, cardiac, and chronic pain disorders by having a thorough understanding of these physiological systems.
