In physiotherapy for first-time patients, it is crucial to check for serious non-musculoskeletal pathologies to ensure patient safety and optimize clinical outcomes. This chapter gives an evidence-based overview of "red flags" in musculoskeletal physiotherapy, focusing on systemic and spinal conditions that seem like normal back pain. The chapter covers conditions like, Cauda Equina Syndrome (CES), Spinal malignancies, Spinal infections (like discitis and osteomyelitis) and Insufficiency fractures, alongside visceral issues that mimic musculoskeletal dysfunction. The chapter advocates moving from just check-list based interpretations to probabilistic framework. This approach combines checking for flags with clinical judgment, context and symptom patterns to minimize false - positive alarms. The chapter emphasises on importance of an approach to patient screening. Furthermore, it outlines clear clinical decision pathways for when to urgent emergency medical referrals. By connecting clinical recognition with emergency care this text helps advanced practitioners, with diagnostic utility and systemic communication strategies required to keep the patient safe and navigate complex diagnoses effectively.Published
